Reablement Worker

Reablement Service

£13.09 per hour / £24,581 pro rata

Hours: 30 Per Week

Golden Hello: £1500 (£500 upon start, £1000 upon completion of probational period)

We are looking for caring and self-motivated individuals who have a passion for improving people’s lives to join our highly reputable Reablement service. This service has been running in Sefton for a number of years and is highly reputable within the Sefton community; we support and work in partnership with the NHS and Sefton Council directly, to enable people to live independently at home for longer.

Being a Reablement Health Care Worker is so much more rewarding than you think. In this role, you aren’t taking care of people in a healthcare setting – you are helping them to manage their own personal care within their own homes. By focussing on abilities and outcomes, you are assisting them to improve their long-term independence.

Trusted Assessor

Magdalen Square, Netherton

36 hours per week

£28,450 – £31,010 pro rata

Closing Date: 3rd May 2024

We are looking for the right person to train as a Trusted Assessor and join our team. We will need you to participate in a 5/7 rota, cover the duty phone during evenings and weekends be able to fulfil the travel requirements of the post and be willing to complete a Trusted Assessor award in the provision of community equipment.

The role is exciting and at times fast-paced, so you need to be someone who is a good time manager, self-motivated, adaptable, and able to develop relationships quickly.

You will be responsible for setting goal focused plans with service users, usually in their own home. These will then be reviewed by you within a set timescale.

Support Worker

Our South Hub is a day service for adults who have additional support needs due to learning and/or physical disabilities or are on the autism spectrum. Situated at the heart of Netherton, on Dunnings Bridge Road, it is proud of its extensive grounds with an attractive garden and growing area.

We aim to provide a caring and friendly environment and enable the people who attend our service to keep physically active and stimulated by social interaction and activities centred around individual interests and aspirations

The successful candidate will participate in the day-to-day delivery of a range of services for people with learning disabilities, physical disabilities, autism or associated mental health conditions; working with service users in a person centred way to promote greater choice, skill development, independence and inclusion. Contribute to meeting the assessed care needs of service users in a way that respects the dignity of the individual and promotes independence, demonstrating a sensitive, understanding and patient approach.

The successful candidate will provide a programme of personal and social care, according to a Care Plan, specific to each individual service user. Care duties will include personal care, dietary care and a range of community-based daily living tasks and therapeutic activities. You will also work with service users, carrying out all aspects of their Care Plan to achieve agreed outcomes.

Team Leader

James Dixon Court, Netherton

An exciting opportunity has arisen to join our team at James Dixon Court. Working as part of an integrated team delivering care and housing-related support in what was traditionally a 30-bed residential home. Recently, we have changed the focus of the service and redesigned it into a reablement bed facility to meet local demand. The primary focus of this service is to enable people to reach their maximum potential which will increase the opportunity to remain living independently in their own home.

You will contribute to meeting the assessed care needs of people we support in a way that respects the dignity of the individual and promotes independence, demonstrating a sensitive, understanding and patient approach.

The post holder will assist the manager in the day to day running of the service, ensuring an environment is provided where people we support feel secure and comfortable. You will be responsible for supervising the daily work routine of Care Workers, ensuring that they are meeting the requirements of the post.
